Required Documents
The applicants aged under 21 years are required to submit the following documents.
- 2 Photographs (Passport size)
- 1 CNIC or B-Form copy of the applicant
- One of the Parents (Father or mother) CNIC copy
- 1 copy of the parent’s domicile certificate, including the applicant’s name
- 1 copy of matric certificate
The applicant having age above 21 years is required to submit the following documents.
- 2 Photographs (Passport size)
- Occupation/trade proof document
- 1 CNIC or B-Form copy of the applicant
- One of the Parents (Father/ mother or husband) CNIC copy
- Last month’s original utility bill of either electricity, gas, or telephone
- Proof of residency in the city, for example allotment letter
- NOC of the applicant from the native district if he/she has the dual address on CNIC.
- If the applicant is married, then a marriage registration certificate is also required
- 1 copy of matric certificate

Step 1:
First, applicants must submit the fee deposit challan of PKR. 200 in the National Bank of Pakistan or State Bank of Pakistan.

- Original CNIC and 1 copy attested copy of it. If the applicant is under 18 years, then submit the attested copy of the B-Form.

- Children aged under 21 years need to submit the attested CNIC copy of their parents(mother or father).
- If the applicant is an employee, then 1 attested copy of their service.
